Target note

Full name: Phospholipid ethers (alkylphosphocholines)
Nature: No single receptor: these lipid analogues accumulate in the lipid rafts of tumor cell membranes and are retained there for a long time.
Normal expression: Low retention in healthy tissues; slow clearance.
Disease expression: Broad spectrum: Waldenström macroglobulinemia, myeloma, lymphomas, pediatric solid tumors, head and neck cancers.
Value: "Pan-tumor" targeting independent of antigen expression.
Approaches: ¹³¹I-iopofosine (CLR 131), ⁹⁰Y-NM600, ¹²⁵I-CLR 125 (Auger).
Indications explored: Relapsed Waldenström disease (phase 3 in preparation), refractory pediatric tumors.
Points of caution: Myelotoxicity linked to prolonged circulation.
