Therapy compass

Targeted radionuclide therapy (radioligand therapy): clinical trials and approved products, arranged by target (sectors). Rings run from approved products at the centre to phase 1 at the edge; each dot is a trial (or a molecule), coloured by radionuclide. Click a sector, a ring, a dot or a diamond.

Verdict

Published articles (PubMed) reporting the results of prospective trials, summarised from their abstract. "Positive" and "negative" refer to the primary endpoint; "early" means a phase 1 trial whose objective is safety. The radionuclide, emission and search filters also apply here.

Sources: ClinicalTrials.gov (API v2) and the EU Clinical Trials Information System (CTIS). Scope: interventional phase 1 to 3 trials using a targeted therapeutic radionuclide (and ²²³Ra) that are recruiting, not yet recruiting, active or completed since 2023. A trial registered in both registries is counted once. Target, vector, radionuclide and indication are classified automatically, then reviewed for code-named molecules; unpublished targets are filed under "Undisclosed".

Approved products: manual review of FDA and European approvals (and of access in France when documented), by indication. A molecule approved in one indication can still be in phase 1 to 3 trials in others: it then appears both at the centre (diamond) and on the trial rings. The "Molecules" view and each molecule's page show this path indication by indication.

Importance score (out of 100): phase (up to 30), design (randomized 12, comparator 5, blinded 3), enrolment (up to 20, log scale), international reach (up to 10), industry sponsor or cooperative group (8 or 5), primary endpoint of survival or progression (12) or response (7). It measures the likely weight of a trial, not its quality. Pivotal trial: a trial whose results support a marketing application (VISION for Pluvicto, NETTER-1 for Lutathera); "pivotal (approval)" when it actually supported an approval, "likely pivotal" for a randomized, industry-sponsored or 300+ patient phase 3.

Territories: for each product, approval in the United States (FDA), the European Union (centralised or national marketing authorisations), France (marketing authorisation, or access programme supervised by ANSM) and Japan (PMDA). The "Approved in" filter keeps only products approved in the chosen territory. Japanese approvals and older national authorisations may be incomplete.

Patents: for approved products, last listed patent and exclusivities in the United States (FDA Orange Book, via Pharsight) and European regulatory protection (8 + 2 years after centralised authorisation). European patents, supplementary protection certificates and litigation are not covered: these dates show a trend, not the certain date of generic entry.

Limitations: Chinese and Japanese registries are not searched; preclinical work is not included; statuses are those declared by sponsors. Earlier years of the chart include trials whose status is no longer updated.
