About Theranostic Atlas

An open atlas of radiopharmaceutical development: clinical trials and approved products for targeted radionuclide therapy and PET/SPECT imaging, arranged by target and phase and updated every month.

What the atlas shows

Two interactive compasses. Each sector is a target (PSMA, SSTR, FAP…); rings run from approved products at the centre to phase 1 (and, for imaging, studies without a phase) at the edge; each dot is a trial, coloured by radionuclide. Every trial links to its registry record, and each target has a reference note (biology, expression, value, approaches, points of caution).

• Therapy compass: targeted radionuclide therapy (radioligand therapy) with ¹⁷⁷Lu, ²²⁵Ac, ²¹²Pb and other emitters.
• Imaging compass: diagnostic PET and SPECT tracers in oncology, neurology, cardiology, infection and endocrinology.

Method

• Sources: ClinicalTrials.gov (API v2) and the EU Clinical Trials Information System (CTIS), queried automatically on the 1st of every month. A trial registered in both is counted once. Published results come from PubMed; approvals from the FDA, the EMA, national agencies (ANSM in France) and the PMDA in Japan; US patents and exclusivities from the FDA Orange Book via Pharsight.
• Scope: interventional trials, recruiting, not yet recruiting, active, or completed since 2023. Therapy: phase 1 to 3 trials of a targeted therapeutic radionuclide (and ²²³Ra). Imaging: trials with a diagnostic purpose using a PET or SPECT tracer, including studies without a phase.
• Classification: target, vector or tracer class, radionuclide, indication and clinical domain are assigned automatically from the registry fields, then reviewed for code-named molecules. Trials whose target is not public are filed under "Undisclosed".
• Importance score (out of 100): phase, design (randomization, comparator, blinding), enrolment, international reach, sponsor type and primary endpoint. It estimates the likely weight of a trial, not its quality.
• Monthly history: each update records new trials and changes of phase or status, shown as "new" on the wheels.
• How it is made: the classification rules, the reference notes and the summaries of published results are written with the help of AI (Claude, Anthropic) under the supervision of a hospital radiopharmacist. Errors remain possible.

Limitations

Chinese and Japanese registries are not searched, preclinical work is not covered and statuses are those declared by sponsors. Many academic imaging studies are never registered. Patent dates show a trend, not the certain date of generic entry. The atlas is a horizon-scanning tool with no regulatory value: it is not medical advice. For any decision, refer to the summaries of product characteristics, the registries and the publications.

Corrections are welcome: a missing trial, a wrong target, a new approval, a published result.

Theranostic Atlas is designed and maintained by LaRadiopharmacie.fr, the pseudonym of a French hospital radiopharmacist. It is an independent, non-commercial project: it does not represent any hospital, professional society or company, and receives no funding.
