Target note

Full name: Norepinephrine transporter (NET, SLC6A2 gene) — targeted by MIBG
Nature: Membrane transporter for catecholamine reuptake; MIBG, a guanethidine analogue, enters through it and is then stored in neurosecretory granules (VMAT).
Normal expression: Sympathetic nerve terminals (heart, salivary glands), adrenal medulla, liver.
Disease expression: Neuroblastoma, pheochromocytoma and paraganglioma, some carcinoid tumors, medullary thyroid cancer.
Value: Imaging of neural crest tumors and selection for ¹³¹I-MIBG therapy.
Approaches: SPECT: [123I]MIBG (AdreView); PET: [18F]MFBG, [18F]LMI1195 (flubrobenguane), [11C]hydroxyephedrine.
Indications explored: Neuroblastoma, pheochromocytoma and paraganglioma; in cardiology, sympathetic innervation.
Points of caution: Same drug interactions as in therapy; thyroid blocking with ¹²³I.
