Target note

Full name: HER2 (ERBB2)
Nature: Receptor tyrosine kinase of the EGFR family.
Normal expression: Low epithelial expression (breast, gastrointestinal tract, kidney), heart.
Disease expression: HER2+ breast cancers (about 15 to 20%), gastric and gastroesophageal junction cancers, more rarely bladder, colon, lung.
Value: A target validated by antibodies and antibody-drug conjugates; radioligand therapy aims at tumors that have become resistant to these treatments.
Approaches: Antibodies (trastuzumab), affibodies and nanobodies (ABY-271, RAD202, 2Rs15d), thorium conjugates (BAY 2701439); ¹⁷⁷Lu, ²²⁷Th, ¹⁸⁸Re.
Indications explored: Metastatic HER2+ breast cancer, gastric cancers (phase 1).
Points of caution: Small proteins (nanobodies, affibodies) are strongly retained by the kidney.
