Target note

Full name: Gastrin-releasing peptide receptor (bombesin receptor)
Nature: G protein-coupled receptor.
Normal expression: Pancreas (high physiological expression), gastrointestinal tract, central nervous system.
Disease expression: Prostate cancer (especially early, low-grade and hormone-sensitive), hormone receptor-positive breast cancer, gliomas, lung cancers.
Value: Expression is often the inverse of PSMA: an alternative for prostate tumors with low PSMA and for HR+ breast cancer.
Approaches: Peptide antagonists (NeoB, RM2, SB3, DOTAM-GRPR1); ¹⁷⁷Lu, ²¹²Pb, ²²⁵Ac.
Indications explored: HR+/HER2− breast cancer (with ribociclib), prostate, glioblastoma, solid tumors.
Points of caution: Pancreatic exposure; agonists caused gastrointestinal side effects, hence the use of antagonists.
