Target note

Full name: Delta-like ligand 3
Nature: Atypical Notch pathway ligand, normally intracellular, exposed at the surface in high-grade neuroendocrine tumors.
Normal expression: Almost absent from the surface of healthy cells.
Disease expression: Small cell lung cancer (more than 80%), extrapulmonary neuroendocrine carcinomas, prostate cancers with neuroendocrine differentiation.
Value: A target validated by tarlatamab; radioligand therapy aims at aggressive tumors where SSTR is often weak.
Approaches: Antibodies and fragments (ABD147, SC16), small proteins (MP0712); ²²⁵Ac, ²¹²Pb, ¹⁷⁷Lu.
Indications explored: Small cell lung cancer, high-grade neuroendocrine carcinomas (phases 1-2).
Points of caution: Efficacy data still preliminary.
