Target note

Full name: Chemokine receptor CXCR4
Nature: G protein-coupled receptor for CXCL12, involved in keeping cells in the bone marrow niche.
Normal expression: Hematopoietic cells, lymphocytes, bone marrow, spleen.
Disease expression: Lymphomas (including primary CNS lymphoma), myeloma, leukemias, small cell lung cancer, adrenocortical carcinoma.
Value: High expression in hematologic malignancies: CXCR4 radioligand therapy is mainly used as myeloablative conditioning before transplant.
Approaches: Pentixather labeled with ¹⁷⁷Lu or ⁹⁰Y, ²¹²Pb-pentixather; selection by ⁶⁸Ga-pentixafor PET.
Indications explored: Refractory lymphomas and myeloma, primary CNS lymphoma.
Points of caution: Bone marrow aplasia expected, hence the need for a stem cell transplant.
