Carcinoembryonic antigen (CEACAM5)
Nature
Membrane-anchored glycoprotein (GPI), also measured in blood as a tumor marker.
Normal expression
Low expression at the apical pole of the colonic epithelium.
Disease expression
Colorectal, pancreatic, gastric and lung cancers, medullary thyroid cancer.
Value
A highly expressed antigen; a testing ground for pretargeting, which separates the antibody from the radioactive payload to limit bone marrow exposure.
Approaches
Anti-CEA antibodies labeled with ²²⁵Ac (M5A), pretargeting with a bispecific antibody then a ²¹²Pb chelate (CEA-PRIT).
Indications explored
Metastatic colorectal cancer (phase 1).
Points of caution
Circulating CEA can capture the antibody; gastrointestinal tolerability to monitor.
