Carbonic anhydrase IX

Nature

Membrane enzyme induced by hypoxia (HIF-1α pathway) that regulates extracellular pH.

Normal expression

Mainly gastric mucosa and biliary tract.

Disease expression

Clear cell renal cell carcinoma (loss of VHL makes expression constitutive, more than 90% of cases); hypoxic regions of many other tumors.

Value

Almost constant and homogeneous expression in clear cell kidney cancer, where PSMA and SSTR are of little use.

Approaches

Antibodies (girentuximab, TLX250), small molecules and peptides (DPI-4452, XT771); ¹⁷⁷Lu, ²²⁵Ac.

Indications explored

Advanced clear cell renal cell carcinoma (phase 3 ongoing), combinations with immunotherapy.

Points of caution

Antibodies: long circulation and dose-limiting bone marrow. Small molecules: gastric and intestinal uptake.
