Target note

Full name: Remodelling bone (hydroxyapatite) — not a molecular target of the tumor cell
Nature: The radionuclide binds to the mineral matrix where osteoblastic activity is intense.
Normal expression: Normal skeleton, with higher uptake in remodelling areas.
Disease expression: Osteoblastic metastases (prostate, breast); less suited to osteolytic lesions.
Value: Treats the tumor bone microenvironment and pain; only ²²³Ra has shown a survival benefit (ALSYMPCA).
Approaches: Bone-seeking ions or complexes: ²²³Ra (calcium mimetic), ¹⁵³Sm-EDTMP, labeled bisphosphonates (¹⁷⁷Lu-DOTAZOL).
Indications explored: Castration-resistant prostate cancer with symptomatic bone metastases and no visceral disease; combination trials (enzalutamide, docetaxel, olaparib).
Points of caution: Myelotoxicity. In Europe, ²²³Ra is contraindicated with abiraterone (excess fractures and deaths in ERA 223) and reserved for use after at least two lines.
